Data on duration of KMC per day was measured by bedside nurses on daily basis.Ģ0 eligible mother-infant dyads were studied during implementation period (50 d). A variety of measures (allowing family members including male members during night for doing KMC, making KMC an integral part of treatment order, introducing the concept of weekly KMC champions, etc.) were introduced and subsequently tested by multiple Plan-do-study-act (PDSA) cycles. The potential barriers for prolonged KMC were evaluated using fish bone analysis.
A QI team comprising of nurses, nurse educators, resident physicians and nursing-in-charge of unit was formed. Preterm mother-infant dyads who were admitted in the Neonatal intensive care unit and KMC ward were enrolled in this study. To increase the duration of Kangaroo mother care (KMC) in preterm infants from an average of 3 hours/day to at least 6 hours/day over 7 weeks through a Quality improvement (QI) approach in a tertiary-care neonatal unit. Keywords: Preterm, quality improvement, kangaroo mother care, infant Conclusion: QI improved KMC duration in the intervention phase but for sustaining these efforts structural and administrational changes are required. We faced many system challenges and bottlenecks during this study: less space for doing kmc lack of privacy no separate bed for mothers of out-born babies low nursing and support staff ratio no provision of separate clean apparel for doing KMC. During the sustenance phase, the mean KMC duration decreased to 1.6 hours per week. Results: The mean duration of KMC increased from 2 hours in the baseline to 4.5 hours per week during the intervention phase. A series of interventions were tested: Counselling and educational video sessions procuring KMC chairs, and the involvement of family members.
The study was done in 3 phases: The baseline phase (2 weeks) the intervention phase (8 weeks) sustenance phase (8 weeks). Fishbone analysis was done for finding the root cause.
Methods: We enrolled stable preterm infant-mother dyads with gestational age <32 weeks and SNCU stay for at least 7 days. Background: A quality improvement (QI) project was planned to improve the mean duration of Kangaroo mother care (KMC) from 2 hours to 6 hours per week in very preterm infants over a period of 8 weeks in our special newborn care unit (SNCU).
